AGREEMENT OF UNDERSTANDING WITH SCUBA DIVING SERVICE PROVIDER 

This Agreement brings principal scuba diving (hereinafter – the diving) safety regulations for your 

kind attention. Make sure these regulations are carefully read and understood to guarantee your safety 

while diving. Please fill in your first name, last name, and middle name/patronymic into the 

corresponding fields to confirm that you have read and understood these 

regulations. Please read the document carefully and discuss it before joining. If you have not reached the 

majority age yet, your parent or guardian should sign the document.


I understand that, according to these diving activities, I shall: 

1. Maintain proper mental and physical fitness. Avoid alcohol and drugs prior to and during diving 

activities. Maintain essential diving skills at a proper level, work continuously through training to 

improve them, and brush up my diving and personal safety skills under direct professional 

supervision after long recess, if any. 

2. Be familiar with my diving location or otherwise rely upon official underwater awareness information 

provided by a reliable local source. Postpone or pick a better location if actual diving conditions are 

more severe than I may have used to face before. Avoid activities outmatching my skills and 

experience. Avoid technical and cave diving activities without proper special training. 

3. Employ complete, up-to-date, well-maintained, and reliable gear I am familiar with. Use the gear 

under certified instructor supervision only while mastering it. Check for gear condition and proper 

fit-up prior to every dive. Avoid providing my gear to unqualified divers. Always use full compressed 

air cylinder, BCD, pressure gauge, dive computer (or NDL table, depth indicator, and underwater wrist 

watch), mask, fins, and surface signal device for scuba diving activities. 

4. Stay focused during pre-dive induction, obey and respect commands, and strictly follow dive 

managers' guidelines while training. Recognize additional training is essential for diving activities of 

a specific nature, in different geographical areas, and when resumed after 6 month recess or more. 

5. Adhere to "buddy system" and never dive alone. Plan every dive with my buddy, including 

communications and buddy lost/emergency actions. 

6. Observe non-decompression limits when scuba diving. Plan my diving properly (with tables or 

computer). Limit my maximum diving depth by my skill and experience. Limit ascension speed to 10 

m/min. Make 3 minute or more safety stop at 5 m depth as a precaution. Avoid air flights 1 day after 

the diving session. 

7. Maintain neutral buoyancy while underwater. Maintain positive buoyancy while on the surface or 

resting. Make sure my weight belt is accessible to release it quickly in underwater emergency. Size 

and secure my weights under professional supervision only. 

8. Avoid holding my breath and purge promptly while underwater. 

9. Know, respect, and obey local laws, traditions, and provisions related to diving regulations. 

I hereby confirm that I have carefully read and understood the above Agreement. 

I do realize that these Regulations are essential for my safety, and any failure to respect them may 

jeopardize my health and safety when diving.



STATEMENT OF INDEMNITY 

Please read the following text carefully.

This is to advise you that scuba diving, as well as snorkeling activities, are associated with certain risks. 

This document outlines cases when you may be liable for those risks. 

Hereunder certifies you have read the Agreement carefully and recognize it. 

Please discuss any clause you may not understand with your instructor or a representative of your diving service provider.

 If you are underage, the document must be signed by either your parent or your guardian. 


WARNING 

Diving and snorkeling activities may cause serious injuries or fatalities. 

Diving with self-contained underwater breathing apparatus may cause decompression 

sickness, embolism, and other hyperbaric traumas subject to altichamber therapy. 

Diving and snorkeling activities put a certain physical exertion upon you. You must provide full details on any 

sickness you may have had as well as your physical and mental readiness to a representative of your 

diving service provider. 


INDEMNIFICATION 

Neither the Instructor nor the scuba diving center or the diving club, where training takes place, nor operators/organizers shall be liable for death, personal injury, or material damage caused by your fault or in circumstances arising from your negligence. 

You shall be fully liable for your diving activities if the Instructor or Divemaster of the diving center or 

Eco District Association that provides the training has performed its part of its obligations respectfully and in full and managed to avoid any 

negligence.  I hereby acknowledge that I have received this document, read and understood all its 

clauses carefully, and recognized it prior to joining.
