A pharmacist’s role in Malaysia has evolved from product-orientated custodian towards serviced technologist. The service has evolved from drug compounding to patient care services, which include hospital based-ward pharmacy, ambulatory care-medication therapy adherence clinic (MTAC) and home visit-home medication review (HMR). HMR is a patient-focused process that advocates the optimal and quality use of medication at the patient’s home. It involves systematic assessment of the patient’s medication in order to identify medication-related needs as well as to identify, resolve and prevent any DRP. By the end of this talk, the speaker will share with you the objective, findings and workflow of home medication reviews conducted by geriatric pharmacists in Hospital Kuala Lumpur.

Our speaker is Ms Jacqueline Wong Hui Yi. She graduated from the IMU MPharm Programme in 2009. She has special interest in geriatrics. She has been practicing since 2012. She is currently a geriatric pharmacist in Hospital Kuala Lumpur and is actively involved in the Geriatric MTac and Home medication review programmes.
